Mr. Ford entered our community on June 20th of 2018 with a diagnosis of dehydration and generalized weakness, as well as an altered mental status. Mr. Ford had his right leg amputated below his knee years ago. He never received a prosthetic and stated he did not want to receive one. After meeting with a consultant from South Beach prosthetics, he had a change of heart, and agreed to be measured and fit for a right below the knee (BKA) prosthetic.

After a few weeks of preparation and wearing a shrinker to help mold his extremity to fit the prosthetic, Mr. Ford received his new prosthetic device and began training with the therapy team for ambulation function and mobility. Mr. Ford endured weeks of training with his new leg and is currently able to walk 100 feet with contact guard assist using a rolling walker. He is able to transfer (sit to stand) at that same level which is an outstanding accomplishment.

Mr. Ford has completed therapy and has improved in all targeted areas. He stated he is so thankful for all the support and quality care he has received from our Jupiter team. It has been a pleasure serving George. He has been motivated and has had a positive outlook on his stay at our community. With that attitude, Mr. Ford is able to return home with his new prosthetic leg and the ability to walk again. He stated he is looking forward to going home to live with his loving daughter and granddaughter. He has been so humble and kind to both staff and the other residents, especially his roommate who he built a friendship with. Mr. Ford will be missed by all of our Jupiter Rehabilitation family and we sent him home with well wishes and a warm hug.

Peter is a 57-year-old male that was involved in a motor vehicle accident with a tractor-trailer while he was riding his motorcycle in May of 2018. Peter was horribly injured sustaining a subdural hematoma, pelvic fracture, multiple rib fractures, right hip fracture, sacral fracture, and a right wrist fracture requiring surgery. During his lengthy hospital stay, Peter also developed a sacral decubitus ulcer.

Peter was admitted to Jupiter Gardens for short-term rehabilitation with numerous complications and injuries as a result of his motor vehicle accident. He was restricted to non-weight bearing to both of his lower extremities, as well as his right upper extremity, which had to be immobilized in a sling. Due to the severity of his injuries, Peter required total assistance for all of his self-care needs and was completely bed bound.

Peter received intensive physical and occupational therapy. With many weeks of hard work and perseverance, he was able to independently get in and out of bed, transfer to all surfaces, perform all of his self-care tasks, and eventually walk over 100 feet using a wide based cane. Between Peter’s determination and drive, and the hard work of the therapy staff, he was successful in being able to return home three months after this traumatic accident.

Peter is a local here in Jupiter who loves Harleys. He even shared with us that he worked for Harley for thirty plus years and he loved every moment of it. Peter owns an auto shop where he works on motorcycles and has a good client tell from his years worth of experience of working in the industry. Peter has been riding motorcycles for fifty years!

Peter has two daughters and a son that he cherishes dearly. He was so excited to be discharged from our community in time to see his youngest daughter go off to college. Peter’s oldest daughter is also starting school to be a dental hygienist. We all enjoyed the stories that Peter shared about his children growing up. He shared that he is thankful to be doing so well, and that he can now witness his daughters complete their goals.

Peter kept a positive attitude and stayed consistent with his therapy during his stay at our wonderful community.

As Peter walked out the front doors on his discharge day, he was sent off with a farewell in the warmest way: hugs, handshakes, and big smiles. We wish him the best!

Gloria is a 61-year-old woman who was admitted to Jupiter Rehabilitation and Healthcare Center, with diagnoses of COPD (chronic obstructive pulmonary disease) and hypertension. At the time of evaluation, Gloria showed weakness of her lower extremities with decreased balance and required moderate assistance from sit to stand and while ambulating to the bathroom. After three and a half weeks of intensive physical and occupational therapy, Gloria showed significant progress in all functional levels.

Today, Gloria is preparing to return home. With hard work from herself and the team at Jupiter, Gloria is now independent while walking around the facility with a rolling walker and is also able to maneuver her rolling walker around obstacles. Gloria is able to dress herself and transfer independently. She is also independent with her home exercise program.

Gloria was extremely motivated during therapy sessions, which overall assisted her to reach her goal to now be able to leave our facility in a better state compared to her condition upon admission!

The team would like to shine a light on one of our wonderful residents who has done a spectacular job in our physical therapy department, Ms. Mabel. She is an 89-year-old woman, admitted to Jupiter Healthcare Center on April 10th, 2018, with a diagnosed fracture of the right femur. At the time of evaluation, Mabel was total assistance for bed mobility, transfers, and adult daily living task. This was all due to weakness in her lower extremity with decreased balance. With all of her ailments, we knew that Ms. Mabel had a long road ahead of her; however, we were confident that our team would be able to lead Ms. Mabel back to independent living.

After ten weeks of intense physical and occupational therapy, Mabel showed significant progress at all functional levels. As Mabel is preparing to return home, she has modified independence with bed mobility and transfers, with an increase in safety awareness. Mabel is at supervision for ambulation using rolling walker. Mabel now gets herself dressed in the mornings and is independent in her home exercise program. She has always been very motivated during her therapy sessions, which has overall helped make Mabel’s wish come true of going home to live with her daughter. Mabel stated, “all of the staff here have been very nice and helpful during her entire stay here at Jupiter Healthcare Center, I have no complaints only great memories.”

We want to thank Mabel for showing us how beautiful the rehabilitation process can be. Her dedication and motivation are a true inspiration to everyone within the community. No matter what the situation is, there is always a light at the end of the tunnel. We are happy to guide her.

88-year-old, Clara, was admitted to Jupiter Rehabilitation and Healthcare Center in March of this year following a TIA (transient ischemic attack). She also arrived with a wound to her lower leg that inhibited her independence, including decreased strength and increased weakness. Occupational and physical therapy, as well as wound care, was prescribed to assist Clara reach her goal of becoming Independent.

In the next six weeks, Clara worked daily with multiple therapists in assisting her with all daily activities including dressing herself, transferring from her bed to her wheelchair, standing up to use her walker, and getting herself to and from the restroom as needed. Upon admission, she was functioning at a level that required maximum assistance with all activity. She has progressed in exercises with our physical therapist in the gym daily and become more independent with each walk that was taken. Clara was always enthusiastic about her treatments because “it was the only way she was going to get home to her grandson.” Present day, Clara is able to do her activities of daily living exercises and walking with her walker at a level that requires only supervised assistance, her balance, strength, and coordination maximally improved during her stay.

Clara loves going to church and is looking forward to reuniting with her church family. Clara enjoys playing card games at home with her grandson. She stated that they sometimes play for hours laughing and talking about current events! Clara has one daughter and two grandsons and six great grandchildren. Clara shared that she is looking forward to being home in her backyard watching her great grandchildren play basketball. During Clara’s stay here, her family came to visit her as well as called to check on her which helped with keeping her motivated to complete Physical Therapy. Although we are sad to see her go, we are thrilled that she achieved her goal of returning home with her family. We wish Ms. Simmons the very best from all of us here at Jupiter Rehabilitation and Healthcare Center.

Mr. Smith is a 64-year-old male who was admitted to Jupiter Rehabilitation and Healthcare Center with the diagnosis of acute renal failure and non-healing bilateral lower extremity venous stasis ulcers. Brian lived with his family in a one story home and was independent with activities of daily living (ADL) and functional mobility prior to going to the hospital. When Brian arrived to our community, he required total assist with self care tasks and functional mobility.

Mr. Smith is a veteran. His expedition to join the Navy began when his father was in the Navy. He was a Culinary Specialist, and Brian wanted to follow in the same path. While serving, he developed both his cooking and baking skills. Post discharge, Brian went to school for thirteen weeks in San Diego. He decided to move to the East Coast and went to school in New Port, Rhode Island, and became a security guard. His passion for baking was still strong, working part time as a baker. Brian held both positions for four years.

Brian enjoyed exploring; he held various jobs in different parts of the country. He was fortunate enough to change his job every five years. He worked from resorts, restaurants, bars, and hotels, doing what he loves the most; baking. His last job before retirement was working at a plastic plant. He held that position for 16 years. The company was making cassette player covers, and then switched over to DVD covers. Once Blockbuster and Hollywood Video went out of business, they switched over to making pill boxes for Walmart.

In 2011, Brian retired from the plastic plant. He moved down to Palm Beach Gardens to live with his brother. He resides with his brother, his girlfriend, her mom, a cat, and a dog.

Mr. Smith was admitted to Jupiter Medical Center with a diabetic wound that developed on his leg. He was then recommended to receive therapy and with the help of the case manager he came to Jupiter Rehabilitation and Healthcare Center .

Mr. Smith had a very positive journey during his stay at Jupiter Rehabilitation and Healthcare Center , “I love this place. People are friendly. Aides and nurses are hard working. I have been to other places where everyone is just talking in the hallway, but not here, this place takes the cake. The food is delicious. I will miss that when I get out. I talked with some of the residents here who are very friendly. Then I saw the concierge speaking with a resident and I knew that I just needed to meet him.”

Mr. Smith continued by stating, “I enjoyed therapy, they are all friendly. I was sent here to learn how to walk again. I was thinking about doom, but this place motivated me to think otherwise. Dr. Patel told me that I will walk before I get discharged and he was right. I would like to give a special thanks to Mellissa, Marsha and Debbie from the therapy department. They were able to motivate me to start walking again. They gave me stories that motivated me.”

It has been a long journey with many obstacles, but Brian persevered through it all, making the minor setbacks into a major comeback. With weeks of hard work and determination, Brian now requires minor supervision to complete activities of daily living and functional mobility utilizing a rolling walker. He is thrilled to be able to return home with his family and start baking again.

We are ecstatic that Mr. Smith was able to return home to his family. He worked diligently to get back to the things he loves. From all of us at Jupiter Rehabilitation and Healthcare Center, we wish Mr. Smith the best!

Mark Vadas was admitted to Jupiter Rehabilitation and Healthcare Center on March 14, 2018. He went to the hospital for a right total hip arthroplasty on March 12, 2018. At the time of evaluation, Mark needed moderate assistance with bed mobility and transfers. During gait training, he required minimum assistance and walked a distance of ten feet with a rolling walker. After two weeks of intensive physical and occupational therapy, Mark showed significant progress in all levels of function.

Mark’s first experience with the facility was twelve years ago, when his mother was here. It was hard for Mark to come back, but ultimately decided this was the best place for rehab. The experience he had here with his mom was excellent. He felt comfortable selecting Jupiter Rehabilitation and Healthcare Center , even though he just wanted to go home after the hospital.

Mark’s therapy experienced changed his outlook on going to a rehabilitation center.

“I would recommend therapy for anyone. It transforms you into a rejuvenated person,” shared Mark!

He enjoyed and respected the persistence of the therapy team, specifically Renee, whom he has identified as his “ray of sunshine” while in the facility. Mark received both physical and occupational therapy to strengthen his upper and lower body.

The staff here at Jupiter Rehabilitation and Healthcare Center is excited to see Mark become independent once again. “The therapist gave me the tools to perform many of my own functions and not have to rely on my wife.” He also enjoyed the newly renovated room he was in. As an interior designer, he thinks the color schemes in the room are terrific. He has also asked the chef for some of his recipes, as the dining experience here was magnificent.

Mark is now able to get up from bed and transfer at a modified independent level. He is able to dress himself in the morning with good safety techniques. He is walking with a rolling walker at a modified independent level and is preparing to go home with his wife.

Mark is always very motivated during therapy sessions and has worked hard with his therapist to return home safely. As a member of the coast guard we are honored and proud to serve Mark.

Mr. William Flora was admitted to Jupiter Rehabilitation and Healthcare Center on January 22, 2018. He was hospitalized for Congestive heart failure, Atrial fibrillation, respiratory failure, severe osteoarthritis of the hands and ankles, and suffered from a stroke. The new diagnosis was hard for Mr. Flora to accept because he was always accustomed to doing everything on his own.

William was born and raised in New Hampshire, before moving down to sunny Florida several years ago. He joined the air force for 4 years and worked on B47 bombers as a flight line mechanic. William spent his time fixing air planes during the Vietnam War. Following his service in the air force, he worked as a system analyst professional. Eventually, Mr. Flora changed careers and started a roofing business, then ultimately moving to construction and home building.

“I woke up in bed not knowing what happened to me. I had almost no sodium in my body and went into a stroke. I had to learn how to walk again, which seemed natural and to get hold of my brain and figure who I was again. I committed to eating right and working out daily to get back my strength.”

Initially, William required extensive assistance for bed mobility and transfers secondary to strength deficit of lower extremities and decreased range of motion of the upper extremities. After three weeks of intense cardiovascular rehabilitation, Mr. Flora is now able to transfer to the bed and to the toilet independently with safety using a rolling walker.

Today, Mr. Flora is preparing to return home. He gets himself dressed in the morning, walking with a rolling walker with distant supervision within the facility and is independent with home exercise programs. He looks forward to getting back home to enjoy his grandkids company.
