How did this patient hear about Jupiter? Case Manager at JFK.

Details of experience:

Sabato Adinolfi came to us from JFK Medical Center on August 10th, 2021. He was complaining of shortness of breath that had continuously worsened over the last few weeks. He had undergone coronary artery bypass surgery and mitral valve regurgitation. He was discharged to Jupiter Rehabilitation and Healthcare Center for assistance with all mobility and all activities of daily living.

Mr. Adinolfi came to our community in the height of the covid pandemic, with many limitations being in place. He made it his mission to thrive and work towards going home. When he wasn’t in therapy, Sabato would pass the time playing games on his iPad or with window visits from his family. Through weeks of skilled training and therapy, Mr. Adinolfi was safely discharged to his home on September 9th, 2021. He was able to perform all his activities of daily living and mobility at an independent level! He is most excited to get home to spend time with them and go about his normal daily routine.

We will miss him at Jupiter Rehab.

Admission Date: July 16th, 2021
Admitted From: Encompass Health
Length of Stay: One Month and two days
Reason for Stay: Left Femur Fracture with ORIF
How did this patient hear about Jupiter? He and his wife had heard about Jupiter Rehab from a friend, and it is close to home.

Details of experience:
Mr. John Vogeley came to Jupiter Rehabilitation on July 16th, 2021, from Encompass Health, after sustaining a fall at home resulting in a left femur fracture with an ORIF. When he arrived at our facility, he had toe-touch weight-bearing restrictions and required about 50% assistance to do any mobility or activities of daily living (ADLs). Over the next four weeks, working with skilled rehab, Mr. Vogeley was able to return home with his wife and perform ADLs at a modified independent level without any need for physical assistance.

John Vogeley isn’t a man of many words. He had a sense of calm that made it very easy to be around, and it was comforting to sit and talk with him. With a strong team awaiting his return home, John’s biggest motivation was to be home with his wife and to be able to play cards with her and their son. John started playing cards at a young age and has carried the love of card games with him since his wife even brought him some cards with larger writing so he could play solitaire to pass the time while at Jupiter Rehab.

The staff will miss Mr. Vogeley; he was a pleasant individual and always up for a nice conversation. We wish Mr. Vogeley and his family all the best.

Reason for Stay: Partial Tear of the Left Calf Muscle
How did this patient hear about Jupiter Rehabilitation? Her daughter’s coworker referred them to us, her coworker’s mother had stayed with us and had a wonderful experience.

Details of experience:

Lindsey Bamberg is a 76-year-old female who was admitted to Jupiter Rehab on May 26, 2021, from Boca Raton Regional Hospital. She was admitted to the hospital on May 23 with complaints of left calf pain and swelling, an MRI of her leg showed soft tissue swelling, and she was diagnosed with partial tearing of her left calf muscle. She was advised to go to a rehab facility to help with the rehab. Mrs. Bamberg and her daughter chose Jupiter Rehab after a referral from a coworker and the fact that her daughter lives close by. When she was first admitted, she required minimal assistance for her mobility and self-care. After weeks of physical and occupational therapy, Mrs. Bamberg performed all her mobility at supervision level, walking about 100-150 feet using a rolling walker and performing all self-care tasks independently. She was safely discharged to her home.

If you ever had a down day, Lindsey Bamberg’s smiling face could make all your worries disappear with just a smile. Her positive attitude and appreciation for her care radiated throughout the building. There wasn’t a staff member or fellow resident that she didn’t make feel special. People were drawn to her. After getting to know her better, I learned that she had been a CNA for years! No wonder she was so good with people!

You could tell that Mrs. Bamberg felt well taken care of…not only did she have high praise for her physical and occupational therapists, she also quite enjoyed our Activities Department. “The dining room sessions were awesome, and they kept morale high!” and she felt that our kitchen was top-notch, “The food is delicious and quite varied in items, the kitchen personnel is so accommodating when I make special requests.”

Although hesitant to leave our facility, when she is discharged, Mrs. Bamberg is excited to get home and continue focusing on her physical therapy, sleeping through the night without any interruptions, and just getting back to her beloved hobbies, travel socializing/entertaining family and friends.

Lindsey Bamberg is a model patient, but what we will miss about her most is her warm smile and glowing personality, and we wish her good luck returning home.

How did this patient hear about Jupiter Rehabilitation? Return patient from two years prior

Details of experience:
Mr. Runnels came to us from Jupiter Medical Center on May 31st, 2021. He says it was an easy decision to choose Jupiter Rehabilitation & Healthcare Center because it is just down the road from his home and wife, and he knew he would be in good hands for his rehabilitation journey. Robert (Bob) had been with us two years before this stay and said that the success he had with his prior stay made it an easy decision. He was admitted to us s/p quad repair from a fall he sustained at home, leaning over to pick up a paint can that he had dropped trying to throw away. Bob joked that had he heeded his doctor’s orders after his first stint at Jupiter Rehab, he probably would not have ended up with us again.

Upon admission, Bob was unable to perform any mobility independently, including his activities of daily living (ADLs). He received PT/OT 5x’s a week with great motivation and positivity. The positivity he carried with him throughout the facility, always being able to put a smile on everyone’s face. His highlight of being with us at Jupiter Rehab was his roommate. He and his roommate gained a special rapport laughing, bantering, and joking with each other and the staff.

Upon discharge, Bob returned home with his wife and was able to perform all his mobility and ADLs with supervision. He is looking forward to spending time with his wife, going out to eat with friends, and spending time out in the yard tinkering with things.

We will miss having the positive and uplifting presence of Mr. Runnels, but we wish him the best and hope to never see him here again at Jupiter Rehabilitation & Healthcare Center!

Details of experience:
Michele was staying at Heartland Healthcare receiving therapy after falling and fracturing her femur. During her stay at Heartland, she suffered a stroke. Prior to her stay, Michele was a very independent person living on her own. She lived a very active life, so it was difficult when she suffered a stroke. She served our country in the Army, and not being able to be mobile and do things for herself was difficult.

Michele arrived at Jupiter Rehab on November 20th, 2020. At the time of her admission, she required maximum assistance for her activities of daily living, using the restroom, bed mobility, transfers, and ambulation. She worked hard every single day during her therapy sessions. She would even perform the exercises that the therapists would do with her, on her own, to help strengthen herself. Michele extended her stay twice. She felt that our therapy team was so great and supportive. She wanted to stay to receive as much help and learn from our therapy team. She wanted to make sure that she was strong enough prior to discharge. After weeks of hard work, countless obstacles and dedication, Michele was able to be discharged! At her time of discharge, she was modified independent level for all her activities of daily living and mobility.

While she was with the community, Michele enjoyed her stay immensely. She loved the staff, and she thought they were nice and caring and enjoyed being able to get to know them. She even became really great friends with one of her roommates. They were always laughing and having a good time, it was so nice to see their happiness. They exchanged numbers so that could stay in touch after her roommate left. Michele was such a pleasure to have and it was great seeing how she improved every day.

The team is so proud of the hard work she put in and to see her walk out of our building and go home was such a great feeling! They miss her and wish her all the best!

How did this patient hear about Jupiter Rehabilitation? A case manager from Palm Beach Gardens Hospital

Details of experience:
Isabelle’s story starts the week of Christmas, that’s when she had her first fall. She was sitting on the couch enjoying the day, getting ready to enjoy the holidays with her family, and the next thing she knew she was on the floor. Thankfully Isabelle was okay and didn’t need to go to the hospital. A week later Isabelle was sitting in the living room in her favorite chair, and she slid off the chair and fell onto the floor. This was her second fall in a week, after this fall she started experiencing hip pain. The pain got so bad that she had to transport to the ER, thinking it could be something more serious after her fall.

Isabelle was taken to Palm Beach Gardens Hospital on December 29, 2020. She was admitted and they took x-rays, performed a CAT scan and an MRI. She was very fortunate, that she did not have any fractures in her left hip and did not need surgery!

Isabelle arrived at Jupiter Rehab on January 1st, 2021. When she arrived, she needed substantial help for mobility and activities of daily living. She was very excited to start her therapy sessions when she arrived, she was very motivated. After a couple of weeks of therapy and hard work, Isabelle was able to perform ambulation, transfers, and mobility at a supervision level.

Isabelle truly enjoyed her stay, she was always happy and loved the staff, and was always saying how nice everyone is. She became friends with her roommate, Michelle, and they were always laughing and having a good time. We were sad to see her go, she was such a bright person, but the team is very happy that she was able to go back home!

Team Jupiter misses her and wishes her all the best!

Details of experience:
Imagine a beautiful, sunny day in Florida and you decide to take a ride on your motorcycle. You’re riding down the open road, feeling the warm Florida breeze on your face and the next thing you know you’re lying on the concrete. Unfortunately, this scary accident was John’s. He was extremely fortunate that he was not more severely hurt, considering he was not wearing a helmet!. Believe it or not, this was not his first motorcycle accident, this was his third! His first crash was in 2009 and he suffered left-sided rib fractures and left clavicular fracture, and in 2012 he suffered nasal bone fractures and facial lacerations. John was flown by helicopter to St. Mary’s Medical Center. Upon arrival as the staff was examining him, they saw that he had a deformity to his right femur with an open wound, fractured scapula and dislocated 2nd, 3rd, 4th, and 5th MTP joints.

On October 17th, John was taken to surgery to repair his extensive injuries from his motorcycle crash. He was at St. Mary’s Medical Center for 3 weeks. He arrived at Jupiter Rehab on October 31st. When he arrived he was non-weight bearing with his right upper extremity and right lower extremity. John required max total assist for all functional mobility and activities of daily living.

John worked hard during his therapy sessions, six days a week! He looked forward to his therapy sessions because he wanted to be able to get strong and get back on his feet. He worked hard for 9 weeks! His hard work paid off. At the time of his discharge, John had progressed to limited supervision for all his mobility and activities of daily living!

We are so proud of John and how hard he worked to get better, we wish him all the best!

How did this patient hear about Jupiter Rehabilitation? She was a previous patient and a family member is an employee.

Details of experience:
Grace was having a relaxing morning, enjoying her breakfast when she heard a knock on her door, she got up and went to answer the door only to find that there was no one there, she started to back up to go back inside when she tripped and fell down on her right side. She was taken to Jupiter Medical Center, where they diagnosed her with a fractured right hip. On August 26, 2020, Grace had surgery to repair her fracture.

Grace arrived at Jupiter Rehabilitation and Healthcare Center on August 31, 2020. When she arrived, she was unable to ambulate and she needed assistance with all her activities of daily living and transfers. The interdisciplinary team worked closely together to create an individualized treatment plan for Grace to help her maximize her outcomes. She worked hard during her therapy session, maintaining a strong focus on her long term goals. Within the first couple of weeks, Grace progressed in all of her short term goals. Prior to her discharge, Grace was able to ambulate and participate in all of her activities of daily living independently.

Grace felt emotionally supported by not only her family member who worked in the community but the special relationship she created with her roommate. They both had a great time together. Despite how much she enjoyed her stay at Jupiter Rehabilitation and Healthcare Center, she was very happy to go home to her dog.

Team Jupiter was honored to be a part of her recovery and wishes her all the best!

Details of experience:
Marguerite H.’s story begins on February 28, 2020. Marguerite lives with her husband, Robert, in their home of 40 years. In that early morning in February, Marguerite fell and was found by her husband. He didn’t know the reason for the fall; and she could not recall, as she suffers from dementia. Robert called 911 and Marguerite was taken to Jupiter Medical Center. Upon arrival, she complained of ankle pain. She was transported to radiology for an x-ray of her ankle. The x-ray showed that she fractured her ankle and that the fracture was serious enough to require surgery.

Mrs. H. arrived at Jupiter Rehabilitation and Healthcare Center on March 6, 2020. She was a non-weight bearing on her right leg and required extensive assistance for all mobility and transfers. Additionally, she required assistance with her activities of daily living, such as bathing and going to the restroom.

Marguerite worked hard, week after week, with therapy and she progressed every week. After all the intensive therapy, the patient was able to perform all mobility and activities of daily living at the supervision level. She was given the all-clear to return home with her husband, Robert, on June 2, 2020.

Marguerite enjoyed her time here at Jupiter and celebrated 65 years of marriage with her husband! Robert came and visited her every single day, even when COVID precautions restricted visitation, he would come for window visits each and every morning.

Team Jupiter wishes her all the best and hopes she’s doing well!
